Our experienced team will work with you, your family or carer to develop an individualised care plan that supports your needs and helps you achieve your personal goals. We provide services through either:
- Home Care Packages
- Commonwealth Home Support Program
- Full Fee for Service Options
Services may include some or all of the following:
Assistance with basic domestic tasks such as vacuuming, mopping, cleaning the bathroom while encouraging and skilling people to do those tasks they can do themselves.
Linking people with activities including shopping, banking, libraries, clubs, coffee shops, community centre programs and group outings. Transport to access medically related appointments.
Assistance with grooming such as showering, hair washing, nails and dressing or just being around to give a sense of security
Keeping people company and helping them to participate in activities at home or in the community while families or other carers have some time out.
Nursing and Allied Health Support
Nursing and allied health support can be provided if assessed as a need; this can include assessment, wound care, treatments, occupational therapy, physiotherapy, body movement and podiatry.
Assessment for providing and assistance in using continence aids and appliances such as disposable pads. Assistance with applications via MASS program.
Nutrition, Hydration, Meal Preparation and Diet
Assistance with preparing meals, including special diets for health, religious, cultural and other reasons, assistance with using eating utensils and assistance with feeding.
Assistance to keep a person’s home in a safe and habitable condition. (Examples: window cleaning, garden maintenance, lawn mowing, rubbish removal, gutter cleaning).
Counselling Support, Information and Advocacy
Group counselling activities, support groups and workshops. We can help by providing information on a range of areas including Health and wellbeing and emotional support, grief and loss issues, practical problem solving techniques and managing stress.
